Dear Friend,

Welcome! Congratulations on taking this step toward supporting your health with nutrition during your cancer treatment and beyond. I hope you have had a chance to browse through this site to learn more about my services (FAQ, services, and home). Changing your diet and taking supplements requires a commitment and effort on your part. If you are willing to make that effort, we will be a great team (click here to learn more). Our goal will be to educate and empower you to support your innate healing capacity using nourishing foods, healing herbs, and dietary supplements appropriate for your needs.

Here is a checklist of the materials you will need to submit in order to become a client. Print the PDF files (click here if you need help downloading/printing the PDF files)

NOTE: Your file will not be considered complete until we have received the five items listed below.

  1. The appropriate Intake Questionnaire, which describes your medical and health history:
  2. A Food Log for you to keep a 2-day record of your daily diet
  3. Copies of medical records (e.g., pathology reports, MRI or CT reports (do not send films), recent blood work)
  4. A photograph of yourself (pre- or post-diagnosis), which helps me get to know my clients better
  5. A signed and dated Consent Form, which also provides your payment information

When you have collected all the appropriate materials, please send them to us via:

When we have received your completed intake forms, I will research and develop a comprehensive written report for you (with handouts, product recommendations and scientific references) to educate you about the diet changes and supplements that have published studies supporting their usefulness for your type of cancer. The report will help you prepare so that we can make the best of our time together during your appointment (its also a handy reference guide). During our telephone consultation, we will review the research with you, discuss how you can implement the protocol and answer any questions you may have.

Follow-up appointments can be scheduled as needed to monitor your progress. Most clients prefer the convenience of telephone appointments, but you are always welcome to travel to Northern Utah for an in-office appointment.
